Same thing happened to me old, reason I don't play the server much anymore.

yeah, it kinda sucked. but it's all good and hopefully it doesn't happen again.

I've never died completely for no reason. But one time I logged out in a tunnel, and when I logged back in, I was inside the wall next to the tunnel and barely managed to get out before I suffocated.

yeah, that's pretty common i think. I always make sure there is at least a one block cushion all around me before i log out

From what I can tell you choked in the tree you logged out below ^^
